Introduction

The project aims to address the significant health risks posed by the high prevalence of obesity, sleep apnoea, and associated cardiovascular diseases in our community. While these conditions are often managed by individual doctors, there is a lack of coordinated community-level effort to screen, educate, and support at-risk individuals. This project, funded through a Corporate Social Responsibility (CSR) initiative, will bridge this gap by creating a comprehensive program that focuses on prevention, early detection, and lifestyle modifications.


Project Objectives


  1. Screening and Early Detection: To screen at least 500 obese individuals for sleep apnoea and cardiovascular risk factors within the first year.

  2. Awareness and Education: To educate 1,000 community members on the link between obesity, sleep apnoea, and cardiovascular outcomes through workshops, seminars, and digital media.

  3. Lifestyle Intervention: To implement a structured lifestyle modification program for 200 high-risk individuals, focusing on weight management, diet, and exercise.

  4. Community Support: To establish peer support groups and a network of trained volunteers to provide ongoing motivation and support to participants.


Target Group


The primary target group is obese individuals (BMI > 30) who are at high risk of or have been diagnosed with sleep apnoea and/or cardiovascular conditions. The secondary target group includes their families and the broader community, to raise general awareness.


Methodology


The project will be implemented in three phases:


Phase 1: Screening and Assessment (Months 1-4)


  • Community Outreach: Organize free health camps and awareness drives in targeted neighborhoods to reach the obese population.

  • Initial Screening: Use standardized questionnaires and portable diagnostic tools (e.g., home sleep apnoea tests) to screen for sleep apnoea.

  • Clinical Assessment: Partner with local hospitals or clinics to offer free or subsidized cardiovascular risk assessments, including blood pressure monitoring, cholesterol checks, and ECGs.


Phase 2: Intervention and Education (Months 5-12)


  • Educational Workshops: Conduct regular workshops with medical professionals (doctors, nutritionists, physiotherapists) to educate participants on the risks of sleep apnoea and the benefits of weight loss.

  • Personalized Action Plans: Based on the screening results, provide each high-risk individual with a personalized lifestyle plan, including dietary guidelines and exercise routines.

  • Support Groups: Facilitate the formation of peer support groups where participants can share their experiences and support each other's journey towards a healthier lifestyle.


Phase 3: Monitoring and Evaluation (Ongoing)


  • Regular Follow-ups: Schedule quarterly follow-up sessions for all participants to monitor their progress and adjust their lifestyle plans.

  • Data Collection: Collect data on key metrics such as BMI, blood pressure, sleep quality scores, and participant feedback to measure the project's impact.

  • Impact Reporting: Prepare a comprehensive report at the end of the project, highlighting the key outcomes, success stories, and lessons learned.


Expected Outcomes


  • A significant reduction in the prevalence of undiagnosed sleep apnoea among the obese population in the target area.

  • An increase in the adoption of healthy lifestyle habits, leading to a decrease in BMI and improved cardiovascular markers (e.g., lower blood pressure, cholesterol).

  • Greater community awareness about the interconnectedness of obesity, sleep, and heart health.

  • Creation of a sustainable model for community-based health interventions that can be replicated in other regions.
